Answer: No, this is a spiritual fall but not death.

Question: How do you feel your actual state as acutely as possible?

Answer: Open your heart.

Question: What is the difference between a sincere confession to oneself of one’s sin, of one’s true intentions, and a confession out loud, for example, to like-minded people?

Answer: There is a big difference. It depends on the depth of realization.
